I have a a question if someone can help me out....?

maybe someone has went through this i have a son by my ex but i gave him my last name because we split up and now im preg. with my new bfs baby but i was just woundering i wanna put my last name and his last name with the baby we r going to name her brailey jade fitzgerald barron but im not sure how to put that down when i get the the hosp. how will it work on the social security card someone please help thank you

to give the baby any other last name than yours you need to fill out an affidavit, the hospital should help

Reply:you can either put it as the first last name being part of the middle name or you could hyphinate the last name to be fitzgerald-barron. What ever is on the birthcertificate when you fill it out will be put on the social security card. Just have it all figured out before you fill out the birthcertificate because it is hard to change that.
Reply:on the birth certificate there will be a line for first middle and last if you have brailey jade then you use the first and middle then on the last you put both but also he signs the certificate and on the social security card it will be just as you placed it on the page except it will use two line of space instead of one like ours good luck and that is very beautiful as she will be i am sure

Reply:My son has both my name and his fathers name, and we never had a problem.
Reply:The child will just have a double last name. It will be treated like a single name with a space instead of a hyphen. Spanish cultures do it all the time
